Children’s Mental Health Targeted Case Management (CMH-TCM) could be the support you’re looking for. CMH-TCM assists children diagnosed with a mental health disorder find and maintain services in the community. Your assigned case manager will assess your child’s needs, work with you to develop a care plan, connect you to community-based services and resources and meet with you monthly to ensure a good fit with providers. What can CMH-TCM help with?
Case management can help you access and provide support with:
- Community-based individual and group mental health services, such as therapy, psychiatry, and day treatment programs
- Social skills and activities
- County grants
- Academic support
- Youth employment programs
- Transportation programs
- County-funded family therapy programs
- Waiver & PCA service
- Medical Assistance programs
- Disability certification

Who qualifies for CMH-TCM?
Reach provides Hennepin County contracted case management services to children who:
- Reside in Hennepin County
- Are ages 5-21 (must complete an intake prior to their 18th birthday but can continue to receive these services
until age 21) - Have a current Diagnostic Assessment with a mental health diagnosis
- Have commercial insurance, are on Medical Assistance, PMAP or underinsured

Success Story
When Aisha and her family first began participating in Reach’s case management program, Aisha had several areas in which she struggled. She often left home without permission, frequently missed school, had poor hygiene, was very argumentative with her mother, and did not have a good knowledge of personal safety. Through the program, Aisha bonded with her case manager and was able to make some significant changes in her life. Aisha began attending school regularly and made lots of friends. She obtained an internship the summer before her senior year of high school. She applied to colleges with the assistance of her case manager and was accepted to each. Aisha continues to thrive from the support she and her family received during a challenging time in their lives.
